TAG Contract Form
Please use the new University of California TAG form, udpated in July 2008, to submit your UC Davis TAG. The previous form will be accepted from those campuses that have already initiated the TAG process. The form is self-explanatory, but please read below for a few advisory notes.
Campus and Major
A TAG contract form should be submitted to each UC campus with which you desire a TAG. UC Davis will not forward the TAG contract to another campus and our representative’s signature on the UC TAG form will validate a contract only for UC Davis.
Please list the correct UC Davis major title in the space provided. Our majors list is provided within this index for your convenience.
Personal Information
Consistent use of name is critical. A student should use the name that appears on his/her community college record. This is the name that will be used on the TAG contract and on the UC undergraduate application.
Name, address, and birthdate must be accurate. This information is used to match the student's application data with the TAG contract during our admission selection process.
Provides a listing of all UC Davis majors. Please use only approved UC Davis major names when writing TAG contracts. Note the following exceptions:

Engineering Coursework
The College of Engineering will honor all engineering articulations as they exist on www.assist.org at the time TAGs are signed in September of 2008. Transfer Credit Agreements, the grids detailing College of Engineering requirements at each community college, are available at the College of Engineering Web site.
Provides a handout that summarizes three basic steps to prepare for UC Davis admission and a one-page summary of lower division course work required for our selective majors.
TAG Publicity
Provides ready-made, easy-to-use publicity for TAGs. Please start NOW to advertise TAGs on your campus. We encourage you to meet with students early so that contracts are written and ready to mail to us on the first day we accept them, September 2, 2008. Remember, TAGs may continue to be written until our designated end date of September 30, 2008 and must be postmarked no later than September 30, 2008.
